We will use this survey to better understand how our co-op can support your family,
address any questions you have, and gather essential information about your family’s needs. This process
helps us ensure that our co-op is a great fit for you and that we have space for your children.
We encourage you to provide as much detail as possible in your responses. Your input is crucial in helping us
determine if this co-op is the right match for your family and in making the co-op experience successful for
everyone.

Regular co-op session days are Wednesdays from 9:30am-2:00pm.
We encourage you to also attend special events, like the Christmas Party and Field Trips.
I understand that this is an on-site parent co-op. If I want to drop my child off in the care of
another parent who is on-site, it is my responsibility to attend co-op until that relationship
between myself and another parent and my child is formed and then I still must get approval from
the Director of the co-op. *
iChoose co-op consists of conservative families who have conservative views. We believe a girl is
a girl and a boy is a boy, that children are magical, created uniquely and wonderfully. We
believe that children need rules and structure, while they also need lots of play, outdoor time,
love, patience, gentleness and care. I understand that iChoose is conservative by nature, but
respects that families have differing views and that if I have a differing view, I should speak
directly with the Director, Ashley Burmley. *
Do any of your children have special needs? If yes, please explain. Please do not avoid this
section and answer truthfully. Special Needs children will not be turned away if we determine
we can meet his/her needs. We have served and do serve families with special needs and
gifted children.

Are you able to pay the co-op registration fees of $40 per family, once a year supply fee of $60,
and monthly co-op dues of $40 per family (up to four children, $10 each additional child)?

Fees and dues all go towards co-op building rental, insurance, supplies, equipment, field trips, etc.
Dues are paid by cash, check or through Venmo. Payment instructions will be provided once you join.

*You enter on the northeast side of the building at the double glass doors. You will be contacted
to confirm your group tour by a member at iChoose Homeschool Co-op.
Children who will be attending co-op are encouraged to attend the tour.
